ENGAGE! Social Prescribing

What Matters to You?

Imagine a more connected Newaygo County—where everyone, regardless of age or background, is valued, respected and included.
At TrueNorth, we believe well-being means supporting the whole person—emotionally, socially, creatively and purposefully. As we age, these aspects become even more vital. That’s why we created ENGAGE! social prescribing — a personalized, non-medical support program for adults 55 and older who are ready to explore what’s next and reconnect with what matters most.

What is ENGAGE!?

ENGAGE! is a social prescribing program designed to walk alongside older adults who may be:

  • Adjusting to retirement
  • Feeling isolated or disconnected
  • Seeking renewed purpose or joy
  • Navigating a life transition

Every journey is different. Whether someone is looking for new structure, friendship, creative outlets or community involvement, ENGAGE! helps guide the way—on their terms, at their own pace.

How Does it Work?

It all begins with a conversation. Participants can be referred by local healthcare providers, community organizations or even refer themselves to connect with a TrueNorth link worker.

Once connected, the link worker—a trained and compassionate guide—reaches out to arrange a meeting. They take time to listen, understand each participant’s interests and goals and help to create a personalized plan for moving forward.

From there, the link worker offers a “social prescription”: a connection to meaningful, community-based activities such as:

  • An art or music class
  • A walking group or wellness program
  • A volunteer opportunity aligned with their skills
  • A support or hobby group
  • A lifelong learning course

These opportunities might be part of TrueNorth’s own offerings or available through local partners. If helpful, the link worker can accompany participants to their first activity, ensuring they feel confident and supported every step of the way.

Who Can Participate?

ENGAGE! is open to adults aged 55 and older living in Newaygo County who want to:

  • Explore new interests
  • Rebuild or expand their social circle
  • Volunteer or get more involved in the community
  • Find joy, creativity, or routine in daily life

What is Social Prescribing?

Social prescribing is a holistic approach to health and well-being. Instead of relying solely on medical treatment, it connects people to local, non-clinical resources that support their emotional, physical and social wellness.

It’s about helping people find connection, meaning and practical support in their communities—often in ways that healthcare alone cannot provide.

Ongoing Support

We know that starting something new can feel overwhelming. That’s why ENGAGE! offers continued support—to help you overcome barriers, build confidence and stay connected.

Your link worker will schedule check-ins to determine if you like your prescribed activity and determine if you’d like to make any changes.

You are always in control of your journey.
