It all begins with a conversation. Participants can be referred by local healthcare providers, community organizations or even refer themselves to connect with a TrueNorth link worker.
Once connected, the link worker—a trained and compassionate guide—reaches out to arrange a meeting. They take time to listen, understand each participant’s interests and goals and help to create a personalized plan for moving forward.
From there, the link worker offers a “social prescription”: a connection to meaningful, community-based activities such as:
- An art or music class
- A walking group or wellness program
- A volunteer opportunity aligned with their skills
- A support or hobby group
- A lifelong learning course
These opportunities might be part of TrueNorth’s own offerings or available through local partners. If helpful, the link worker can accompany participants to their first activity, ensuring they feel confident and supported every step of the way.
